During stimulation, both the stimulating monopolar needle electrode and the lively recording subdermal needle electrode can be adjusted to optimize the response at low ranges of stimulation intensity. Phrenic Motor Study One attainable mechanism in intubated sufferers who fail to wean from the ventilator is dysfunction of one or each phrenic nerves. The phrenic nerves are most often affected as a postinfectious process or as a complication of thoracic surgical procedure. The phrenic motor study could be performed recording the diaphragm with the lively electrode positioned two fingerbreadths above the xiphoid process and the reference electrode sixteen cm from the lively electrode over the anterior costal margin. Finally,ifacentralline is current, the study is contraindicated on the aspect with the catheter (see Chapter43). The phrenic nerve is stimulated in the lateral neck either posterior to the sternocleidomastoid muscle or between the sternal and clavicular heads of the sternocleidomastoid simply above the clavicle.
